Remember to please arrive early so we can complete your initial paperwork. In addition, please remember to bring the following: Current Insurance Card, list of current prescriptions and over the counter medication, your medical and surgical history, recent test results and revelant records, preferred pharmacy name, phone number, and location.
We pride ourselves in making every effort to accommodate same day appointments. Please call no less than 24 hours in advance of any appointment that you cannot keep, so we can make that time available for other patients. Any cancellations with less than a 24 hour notice will be charged a $50.00 fee. In addition, this same fee will apply to the second and additional cancellations.
We are committed to helping you maximize your health benefits. That is why we accept all major HMO and PPO insurances, Health Savings Accounts, Medicare, and self pay patients. In addition, we will bill your insurance company directly for all rendered services so you do not have to hassle with the complexities of claim forms. This includes workman's compensation and motor vehicle accident claims.
